UP AYUSH 2025: The Uttar Pradesh AYUSH counselling authority has released an official notice regarding physical verification of documents for candidates allotted seats in Stray Vacancy Round-5. This verification is mandatory for admission to government and private AYUSH colleges under the state quota.
Candidates whose names appear in the final Stray Vacancy Round-5 allotment list published on the counselling website must report in person at the designated nodal centre with original documents and self-attested photocopies.
Verification Date, Time and Venue

- Nodal Centre: Rajkiya National Homoeopathic Medical College & Hospital

Only those candidates who have been provisionally allotted seats through Stray Vacancy Round-5 counselling are eligible to appear for document verification.
Mandatory Compliance for Allotted Candidates
The authority has clearly stated that reporting for physical verification is compulsory. Candidates who fail to appear at the nodal centre within the specified date and time will forfeit their allotted seat, and the entire security deposit will be confiscated.
Additionally, candidates declared ineligible by AACCC-2025 will not be considered eligible for UP AYUSH Counselling-2025.
Important Guidelines on Reservation & Certificates
- Reservation benefits will be provided only on the basis of certificates issued in the prescribed Uttar Pradesh Government format.
- Reservation certificates from other states will not be accepted for state quota seats.
- For EWS candidates, the certificate must be issued on or after 01 April 2025.
- Only valid and verified certificates will be considered at the time of admission.
List of Documents Required for Physical Verification
Candidates must bring the original documents along with one self-attested photocopy of each:
- NEET UG 2025 Admit Card
- NEET UG 2025 Score Card
- Class 10 (High School) Admit Card (if available)
- Class 10 Marksheet
- Class 10 Passing Certificate
- Class 12 (Intermediate) Admit Card (if available)
- Class 12 Marksheet
- Class 12 Passing Certificate
- Domicile Certificate (if applicable, in UP government format)
- Reservation Certificate (SC/ST/OBC/EWS/Divyang/Ex-Serviceman, if applicable)
- OBC/EWS Certificate issued in prescribed format (valid as per rules)
- One passport-size photograph (same as uploaded during NEET UG 2025)
- Photo Identity Proof (Aadhaar Card / Voter ID / Passport / Driving Licence)
- Printout of online counselling registration and fee payment receipt
Candidates whose Aadhaar authentication could not be completed during registration must carry their Aadhaar card compulsorily for verification.
Advisory to Candidates
Candidates are advised to:
- Reach the nodal centre well within the reporting time
- Carry all documents in proper order
- Ensure certificates are valid and in the correct format
- Avoid last-minute delays, as no extension will be granted
