New Delhi: The National Medical Commission (NMC) has issued a public notice mandating all medical colleges across India to maintain authentic patient records, with immediate effect.
This move is aimed at ensuring transparency and traceability of clinical data in teaching hospitals, critical for both medical education and healthcare accountability.
As per the latest directive, the NMC emphasised that many institutions are failing to implement previously issued guidelines that require recording of ABHA-ID (Ayushman Bharat Health Account) for every patient visiting OPD, IPD, or emergency services.

Mandatory Patient Records
- Medical colleges must document every patient interaction.
- Inpatient records must be authenticated by faculty and senior residents with their names and signatures.
Faculty Validation of Reports
- All investigative reports must be signed off by a faculty member from the concerned department.
ABHA-ID Compulsory
- The ABHA-ID must be recorded alongside hospital registration numbers.
- It is to make sure genuine clinical material is maintained for academic assessments.
Consequences for Non-Compliance
- If any records are found to be fabricated or falsified, disciplinary action will be taken against the concerned faculty and institution.
- NMC reserves the right to conduct verification checks during academic evaluations.
The NMC first issued a circular on June 4, 2024, mandating ABHA-ID as a prerequisite for patient registration, citing its significance in approving increased seat capacities, new colleges, and annual renewals. However, despite clear instructions, many colleges reportedly did not fully comply.
The commission reiterated that no patient should be denied treatment for lack of an ABHA-ID, but only authenticated cases with a valid ABHA-ID will be considered during institutional assessments for the 2025–26 academic year and beyond.
This decision underscores NMC’s increasing focus on data integrity, patient rights, and accountability in India’s medical education ecosystem.
