Jaipur: The Office of the Chairman, UG/PG AYUSH Counseling Board, Government of Rajasthan, has released two separate document checklists – one for Ex-Servicemen/Dependents and another for NRI candidates/Dependents – to be produced at the time of document verification. Both notices are dated September 10, 2026 (No: Cons. Board/2026-27/), and have been issued from the Board’s office at AYUSH Bhawan, Sector-26, Pratap Nagar, Jaipur.
Candidates falling under these categories in the Rajasthan UG/PG AYUSH counselling process must carry the specified documents while reporting for verification.
Documents Required for Ex-Servicemen/Dependents
As per the notice, Ex-Servicemen and their dependents need to produce the following documents during document verification, wherever applicable:
- Application Form
- Discharge Certificate/Discharge Book (IAFY-1940)
- Battle Casualty Certificate
- PPO (Initial and Subsequent PPOs)
- Widow Identity Card
- ECHS Card, Aadhaar Card, Class 10th Certificate of the child, and Dependent Card
- Invalidment Medical Board Proceedings
- Death Certificate
- Ex-Servicemen Identity Card
- Gazette Notification along with relevant Part II Orders, duly signed by the Commanding Officer, for verification of the category of Gallantry Award
- Serving Certificate issued by the Commanding Officer (for serving personnel)
- Certified True Copy (CTC) of the Sheet Roll showing family/dependent details, along with relevant Part II Orders (for serving personnel)
- Release Order/SSC Release Order
Documents Required for NRI Candidates/Dependents
For NRI candidates and their dependents, the Board has specified the following documents to be produced during verification:
- Application Form
- NEET 2026 Score Card
- Valid NRI/OCI/PIO Certificate
- Performa – III
- Performa – IV
- Performa – V – Family Tree
- Passport of Family Tree Persons
