- Two recent passport-size colored photographs attested clearly on the front (To be affixed at the appropriate space on the first page of the Application form).
- One self-addressed envelope (23cm X 10 cm) size affixing postage stamp worth Rs.40/- each.
- Proof in support of rendering service in tribal and backward areas of the State for more than 03 years from the competent authority.
