Procedure For Filling and Submission Of Online Application Form:

The candidates are advised to take a printout of the Prospectus from the official websites and read it carefully, along with the parent/guardian, before filling the online application form for the academic year 2021-2022.
The candidate should log in to Website to get more information regarding the Application Form. For More Information:-

The application forms will be available online from 19.12.2021 to 07.01.2022 upto 5:00 P.M.

I. Candidates have to fill up all the mandatory columns in the online application. All the relevant documents should be uploaded in PDF format. Candidates are instructed to submit the online application after filling up all the required fields.

II. Request for change in any particulars after submitting the online application form shall not be entertained under any circumstances.

III. Candidates are instructed to upload the following certificates while filling the online application form and submit:-

i. NEET-UG 2021 Admit card and Scorecard.
ii. Class X Mark Sheet (both sides of Mark Sheet).
iii. Class XI Mark Sheet (both sides of Mark Sheet).
iv. H.S.C. Mark sheet (both sides of mark sheet) or equivalent
v. Transfer certificate obtained after the completion of H.S.C. or equivalent
vi. Certificate for proof of study from VI STD. to XII STD.
vii. Candidates who are eligible to claim religious and linguistic minority seats in
Self-Financing Medical / Dental Colleges should produce a study certificate
issued by the Head of the Institution last studied -:

Annexure – I (a) or a certificate issued by the competent revenue authority of the candidate’s native jurisdiction –

Annexure– I (b) along with the following certificates:

a) Nativity certificate obtained from Tamil Nadu for candidates seeking admission under the minority category.
b) Permanent Community Certificate Card.
c) Parent Documents are as follows:-

  • SSLC/PUC/HSC Mark sheet
  • Transfer Certificate
  • Non-literate parents should produce First Graduate Certificate.
  • Community Certificate
  • Aadhar Card
  • Driving Licence and Voter ID
  • Ration Card/Passport
    viii. Candidates who are claiming NRI should produce the following documents:-
  • NRI status of the financial supporter issued by the Indian Embassy of the respective country under their seal.
  • Certificate of Relationship between the NRI financial supporter and the candidate issued by the competent authority, with valid Indian Passport of the NRI financial supporter.
  • Candidate’s Passport copy
  • Ward Certificate (Annexure- II)
  • NRE (Non Resident External) Bank Account Pass Book of the
    financial supporter.
    ix. Eligibility Certificate obtained from the Tamil Nadu Dr.M.G.R. Medical
    University, Guindy, Chennai for the candidates of other Universities / Boards
    (except State Board).
    After submitting filled in online application, the candidates are instructed to download
    their filled in online application form and send the photo copies of the above mentioned
    documents in an A4 size cloth lined cover to the following address and the template
    with the requisite details should be printed and pasted on the cover:

    All the entries in the online application form should be filled by the candidates carefully. The candidate should ensure that all information including the marks in the qualifying examination furnished by him / her in the application and the enclosures are correct. The candidates are informed that after proper scrutiny of his/ her application, the marks furnished by him / her in the application are not correct or any other information is found to be incorrect, then;
    (a) He / She will forfeit the admission, no matter at what stage of the course, he / she will be at that time;
    (b) He / She will be debarred from pursuing any other course for a period of three years;
    (c) Legal proceedings / criminal action will be initiated against the candidate along with their parents for furnishing false Mark Sheet / false Community Certificate / false Nativity Certificate.
    V. Even if the candidate is provisionally selected for the MBBS / BDS admission but found to be failed in +1 examinations, the provisional admission of the candidate stands automatically cancelled.
